One billable attempt is one outbound webhook POST that SimpleQ dispatches to your endpoint — whatever the response, including a 429/503/529 backpressure answer. A retry is a separate attempt. What is never billed: publishing (including an idempotent re-publish that returns the existing job), your inbound ack/nack/defer calls, and the backpressure hold itself — waiting costs nothing; only dispatched POSTs count.
